About this Course:  A wellness-based model of nursing care highlights the multiple dimensions of health and acknowledges health as being more than the absence of disease. This type of nursing care is focused on health promotion and illness prevention across the lifespan and within care settings and must simultaneously consider the impact of the social determinants of health (SDOH) on the overall health of clients. The World Health Organization (2018) defines the SDOH as “the conditions in which people are born, grow, live, work and age” (para. 1). Addressing the SDOH and social needs of clients in nursing practice can help reduce health disparities, which are preventable differences in health (i.e., rates of disease, injury, violence, etc.) experienced by populations that are socially and economically disadvantaged. Nurses are well-positioned to address SDOH/social needs and health disparities while delivering wellness-based nursing care focused on health promotion in the home.
